We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.The responsibilities of the Home Care RN include, but are not limited to:Responsibilities:Using the nursing process, provides semi-annual and annual assessments of assigned participants to evaluate home care needs. Records all assessments, reassessments, and care in the electronic medical record.Conducts reassessments to determine ongoing needs and develops and implements a therapeutic Plan of Care with recommendations at least quarterly and whenever a participant's condition changes. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.The responsibilities of the Clinic RN include, but are not limited to:Responsibilities:Conducts initial and quarterly assessments of each assigned participant’s health and social status, identifies overt problems/needs, sets priorities, addresses problems requiring immediate referral and/or follow-up, and interprets and records the latest diagnostic results.In coordination with the Interdisciplinary Team, develops a Plan of Care based on the nursing process that incorporates immediate and ongoing care needs, and includes the perspectives of the participant and their family.Provides care that adheres to accepted practice standards and meets the age-related characteristics and needs of participants.Delivers direct care that includes, but is not limited to, wound care, IV therapy, blood sugar checks, IM injections, vital sign checks, and follow-up on episodic problems previously identified by the Primary Care Physician (e.g., Congestive Heart Failure, chest pain, dehydration, edema, urinary tract infections, or other health concerns).Oversees the organization of the BCSC exam and treatment rooms and coordinates with other Interdisciplinary Team members to schedule participants in a way that ensures day-to-day operational efficiency and participant well-being.When assigned, participates in daily morning meetings to update other Interdisciplinary Team members on changes in participants' conditions and transitions between home, supportive housing, nursing homes, hospitals, and other residential settings. Provides briefings to the Interdisciplinary Team on participant health issues, daily schedules, planned tests, appointments, and scheduled medical specialty visits.Records participant assessments, reassessments, and care provided in the medical record.Consults with and educates the participant, family members, and/or caregivers regarding the disease process, self-care techniques, prevention strategies, and medical interventions.Provides the Primary Care Physician with a summary of health findings and any changes in a participant’s health condition.Ensures that all tests, lab work, and diagnostic studies ordered by the Primary Care Physician have been completed.Coordinates all medication management for BCSC, including stocking non-prescription medications, ordering prescription medications, administering medication in the PACE Center, and monitoring participant compliance. Arranges for the delivery of needed medications and supplies to participants' homes.Collaborates with the Primary Care Physician to ensure protocols and standing orders are followed for bowel care, foot care, skin care, urinary tract infections, vital signs, weight management, and wound care. Provides continence training to participants as needed.Prior to the use of physical or chemical restraints, conducts an assessment, documents that restraint alternatives have been attempted, and obtains a physician’s order for restraints. Obtains family consent or refusal for the use of restraints. Monitors and documents that the participant has a specific medical symptom that cannot be addressed by a less restrictive intervention, and that restraint is required to treat the symptom, protect the participant’s safety, and help the participant achieve their highest level of physical or psychological well-being.Supports the Infection Control function in coordination with the Quality Assurance Coordinator.Actively participates in the Quality Assessment and Performance Improvement (QAPI) process.Recognizes inappropriate participant care management or service delivery by other Interdisciplinary Team members, and addresses these concerns directly or refers them to the RN Clinic Supervisor immediately.Identifies emergency situations and takes appropriate action, including coordinating direct hospital admissions from the clinic to the hospital as necessary.Understands and demonstrates respect for participant rights, and refers ethical issues to the BCSC Ethics Committee.Coordinates with the Pharmacist to support participant care.Coordinates with the Home Care Coordinator to support participant care at home.Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to the Center Manager. Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors, and others.Demonstrates respect for and promotes participant rights, including dignity, self-determination, access to care, confidentiality, and independence.Understands the importance of community involvement and participates, as appropriate, in activities that connect BCSC to its communities.Demonstrates proficiency in delivering age-specific care.Experience and Skills:Degree and Diplomas: Graduate of a school of professional nursing.Certificates and Licenses: RN currently licensed by the California Board of Registered Nursing. Inpatient Social Worker - Geriatric Psychiatry Unit

Documents individual and group interventions daily in the attendance record. Completes progress notes including psycho-social evaluation weekly or more often, as needed.Documentation: Writes progress note on admission, weekly, upon discharge and as needed with new concerns, significant change of behaviors, response to interventions and when new interventions are needed as well as discharge plans. Assures that the patient / patient’s authorized representatives are involved in development of the treatment plan and sign the treatment plan to acknowledge agreement with the treatment plan.Provides discharge planning and documents medical necessity.Assists patient and patient’s authorized representative select an appropriate after care placement when needed.Assist with therapeutic activities, specifically leading 1-2 patient groups per week.Manages unit case load in an efficient manner and coordinates responsibilities with L.C.S.W.Participates in development, review, revision and implementation of policies and procedures for the Acute Psychiatric Unit as needed.Conducts regular unit rounds with LCSW and other interdisciplinary team members as appropriate to identify care and environmental issues and implement corrections as appropriate.Quality Assurance and Performance Improvement: Actively participates in the Acute Psychiatric Unit Quality Assurance and Performance Improvement (QAPI) program. Reviews indicators for social service on a pre-assigned basis, makes any necessary corrections and makes recommendations for improvement to the QA and PI Committee at least quarterly.Assists with coordinating transfers to other acute psychiatric facilities providing involuntary care when needed.Assures security of environment to prevent exit of patients attempting to elope. We serve more than 4,000 seniors across our San Fernando Valley campuses, delivering personalized care tailored to each individual’s needs, from independent living to specialized healthcare programs.The Auerbach Geriatric Psychiatry Unit at LAJH is a leading facility for short-term psychiatric care, offering compassionate, specialized treatment tailored to the unique needs of seniors through a nationally-acclaimed model of high-quality care.The responsibilities of the RN include, but are not limited to: Responsibilities:As assigned, assume the administrative authority, responsibility, and accountability of performing the role and responsibilities of the RN Charge Nurse.Review the daily assignment schedule to ensure each staff member and position is on duty, assigned to appropriate work assignments, and scheduled duty hours. Also, review the supervisory staff assigned for each shift, and check all call-ins for the shift and succeeding shifts. Replace absent personnel in accordance with the department’s staffing policy.Complete M.A.B. certification upon hire and annually thereafter.Ensure a comprehensive assessment is completed upon admission, every 12 hours, and whenever there is a change in behavior, including but not limited to assessing suicidal ideation and homicidal ideation.Ensure that documentation is complete in the medical record, including but not limited to: shift charting, patient/family teaching, individual and group attendances, and MD orders.Communicate with the covering provider and staff regarding changes in patient condition, problems, or clinical concerns. Share necessary information with unit staff.Involve the patient in identifying and effectively utilizing coping skills to maintain safety and improve overall mental and physical well-being.Receive reports from RNs on the prior shift, including changes in condition, scheduled discharges, pending intakes, pending laboratory orders, and safety concerns.Audit and monitor patient reactions to treatment and ensure appropriate documentation of the patient’s condition.Initiate the Patient Care Plans for all assigned patients. Each shift, and as warranted by changes in the patient’s condition, ensure that all care plans are current and reflect individualized patient needs and goals.Observe nursing and therapy personnel to ensure established safety procedures are followed in the use of equipment and supplies.Perform other related duties as needed to meet the needs of the organization and patients.Ability to work a 7 p.m. – 7 a.m. shift.Education and Experience:Must possess a current license to practice as a Registered Nurse (RN) within the State of California.Must have, at a minimum, six (6) months of experience as a Registered Nurse (RN) in an acute care medical-surgical or psychiatric facility. Experience must be recent, within the last 12 months.A minimum of two (2) years of supervisory experience in psychiatric care is preferred.Basic Life Support (BLS) certification required.Assaultive Behavior Training, Management of Assaultive Behavior (MAB) certification required within three (3) months of hire.Los Angeles County LPS Authorization required within nine (9) months of hire.Join Los Angeles Jewish Health and make a meaningful impact in the lives of seniors in our community.
