We serve more than 4,000 seniors across our San Fernando Valley campuses and in the community, delivering personalized care tailored to each individual’s needs, from independent living to specialized healthcare programs.The responsibilities of the Certified Nurse Assistant (C.N.A.)/Med-Tech include, but are not limited to:  Responsibilities:Accurately and safely prepare, administer, and document the oral and/or topical medications that are commonly used in this facility and that may be ordered for residents use by the attending physician.Follow the labeling policies and practices of the facility.Ensure that all medications administered are properly labeled.Assist with all medications charting as directed by the Director of Health Services.Accurately measure, record, and report the vital signs of residents.Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered, including reason(s) for refusal.Assist residents with all applicable ADL’s, grooming, light housekeeping including laundry and toileting. Showers and baths as assigned. At times will assist with the application of elastic stockings, slings or support garments.Assist residents in preparing for medical test activities and social programs. Assists in transporting residents to/from appointments, activity and social programs, etc., as necessary.Observe and report the following: presence of pressure areas and skin breakdowns, all injuries including those of an unknown source, including skin tears. Inform the Director of Health Services or Wellness Coordinator of any changes in the resident's condition.Experience and Skills:Must have current CNA certification by the State of California. Medication Technician CertificatePrevious experience in Long Term Care preferred.Skilled Nursing Facility experience and with residents suffering from Alzheimer’s or dementia, a plus.Verification of CPR (BLS) and first aid training.Background clearance, TB screening and successful completion of a health examination by a Los Angeles Jewish Health Nurse Practitioner or M.D.Flexible with working hours. Documents individual and group interventions daily in the attendance record. Completes progress notes including psycho-social evaluation weekly or more often, as needed.Documentation: Writes progress note on admission, weekly, upon discharge and as needed with new concerns, significant change of behaviors, response to interventions and when new interventions are needed as well as discharge plans. Completes daily documentation of individual and group interventions as directed.Discharge Planning/Utilization Review: Initiates discharge planning on admission and coordinates the discharge process with the patient, patient’s authorized representatives and other resources.Treatment Planning: Assures that treatment plans include appropriate social service interventions within 72 hours of patient’s admission and are updated with each change of condition, change in behaviors and at least weekly. Assures that the patient / patient’s authorized representatives are involved in development of the treatment plan and sign the treatment plan to acknowledge agreement with the treatment plan.Provides discharge planning and documents medical necessity.Assists patient and patient’s authorized representative select an appropriate after care placement when needed.Assist with therapeutic activities, specifically leading 1-2 patient groups per week.Manages unit case load in an efficient manner and coordinates responsibilities with L.C.S.W.Participates in development, review, revision and implementation of policies and procedures for the Acute Psychiatric Unit as needed.Conducts regular unit rounds with LCSW and other interdisciplinary team members as appropriate to identify care and environmental issues and implement corrections as appropriate.Quality Assurance and Performance Improvement: Actively participates in the Acute Psychiatric Unit Quality Assurance and Performance Improvement (QAPI) program. Reviews indicators for social service on a pre-assigned basis, makes any necessary corrections and makes recommendations for improvement to the QA and PI Committee at least quarterly.Assists with coordinating transfers to other acute psychiatric facilities providing involuntary care when needed.Assures security of environment to prevent exit of patients attempting to elope. Share necessary information with unit staff.Involve the patient in identifying and effectively utilizing coping skills to maintain safety and improve overall mental and physical well-being.Receive reports from RNs on the prior shift, including changes in condition, scheduled discharges, pending intakes, pending laboratory orders, and safety concerns.Audit and monitor patient reactions to treatment and ensure appropriate documentation of the patient’s condition.Initiate the Patient Care Plans for all assigned patients. A New Lease on Life Thanks to BCSC PACE
In his role as a staff member of the Activities Department at Los Angeles Jewish Health’s Brandman Centers for Senior Care (BCSC) PACE program, Sergio Anleu has seen firsthand how the program helps its participants thrive. But it wasn’t until his own mother enrolled that he realized how truly transformative BCSC PACE could be.“In 2022, when my mom, Fabiola, was barely 60, she was diagnosed with congestive heart failure,” Sergio says. “She worked as a nanny and was incredibly dedicated to her job, so even though she was having trouble breathing, she didn’t want to take time off to go to the hospital. Fortunately, a friend convinced her; the doctor said that if she had waited one more day, she would have collapsed and died.”For Fabiola, the diagnosis meant a radical lifestyle change. Even with the implantation of a defibrillation device, her heart would be too weak for her to continue working. She would have to retire immediately and dedicate her time and energy to keeping her body as strong as possible.“My entire world shifted overnight: On a Wednesday, I went to work; by Thursday, I learned I’d never be able to work again. I felt psychologically destroyed,” Fabiola recalls.Sergio and his two brothers were beside themselves with worry. Not only was their beloved mother facing an existential health crisis, but she would also require a level of daily support that the three siblings, who all worked full-time themselves, could not provide.“I had worked for LAJH for a number of years, but I was relatively new at BCSC PACE and wasn’t sure whether my mom would be eligible,” Sergio says. “When I found out she qualified, I was so relieved.”PACE (Program of All-Inclusive Care for the Elderly) is a health plan for those 55 and older who want to continue living at home, providing them with vibrant activity centers and dedicated care teams to help them stay healthy, active, and socially engaged.As Fabiola sees it, the care she receives has been nothing short of life-changing.Fabiola and Sergio Anleu“It’s hard to describe how amazing BCSC PACE is,” Fabiola says. “They give me meals and provide my medication, which gets sent directly to my house. They host activities like bingo, arts and crafts, Zumba and music therapy. They take me to the psychologist, where I receive counseling. They make sure I see my cardiologist every three-to-six months. We have outings to go shopping. If I’m not feeling well, they come pick me up so I can get to the doctor right away. And I’ve made friends! It’s all such a blessing.”BCSC PACE also gave Fabiola an emergency call device she can press if she falls when she is home alone, and the staff gets in touch immediately to make sure she is okay.“The support BCSC PACE offers is a game-changer; it gives my brothers and me a sense of security and peace of mind knowing Mom is so well-cared-for,” Sergio says.Three years into her participation in the program, Fabiola says she feels like a different person.“When I was diagnosed, I felt like the world had ended. But BCSC PACE has given me life again, lifting me out of my depression and surrounding me with such beautiful treatment and care,” she says. “I still have physical limitations, but now I have the desire to go on, to spend time with all the friends I’ve made here and enjoy all the things I can do.”Sergio has become accustomed to hearing these types of stories from other BCSC PACE participants and their families, but now they hit home in a different way.“My brothers and I consider ourselves so fortunate to be part of the BCSC PACE community,” he says. “My mom is the person who keeps us close, and without BCSC PACE helping her, I don’t know where our family would be.
