Social Services Supervisor - PACE

We serve more than 4,000 seniors across our San Fernando Valley campuses, delivering personalized care tailored to each individual’s needs, from independent living to specialized healthcare programs.The Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE), provides high-quality, comprehensive healthcare services. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.The responsibilities of the Social Services Supervisor include, but are not limited to: Responsibilities:Supervises the other members of the social work department and daily operations, including overseeing completion of assessments and documentation, assisting with high risk casesWorks closely with the Center Director to hire, orient, educate, supervise and counsel members of the social work team.Oversees the Behavioral Health program including support groups, individual counseling, and family education and partnering with community professionals as needed.Acts as a back up to the Center Director as assigned including leading IDT and managing center needs.Participates in weekly meetings with the Marketing Team to assist in managing prospective enrollee cases and assigns an appropriate social worker.Oversees and schedules meetings with the contracted facilities to coordinate care and servicesAs a member of the Inter-Disciplinary Team (IDT) will participate in IDT meetings and fulfill responsibilities as assigned including initial assessments, care planning and on-going re-assessments of participant care. Duties may include participation as a proxy to provide a briefing to the IDT members for care planning and discussion of a specific participant.Completes participant home visits, as required.Maintains current written progress notes and other documentation on the participant Medical Record including signed and dated documentation for all services and quarterly progress reports.Provides individual and family counseling and develops and leads group counseling sessions, as required.Refers participants and families to appropriate community services and acts as liaison and/or advocate with community organizations for participants.Works closely with participants and family members of caregivers during the disenrollment process, including assisting with referrals for primary care and community services.Encourages effective relationships among staff to support the development and maintenance of a cohesive team.Serves as a liaison between BCSC and other community service providers.Consults with and advises other staff regarding the relationship of social, emotional, and cultural factors to health and medical care, and to the availability of social services in the community.Participates in program and policy development of BCSC Social Work.Understands fiscal issues and is appropriately involved in meeting utilization targets.Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to the Center Manager.  Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors and others.Consistently meets or exceeds BCSC targets for productivity, customer service, quality assessment, and performance targets.Continually seeks better ways for delivering services and communication with participants.Understands the importance of community involvement and participates as appropriate in activities that link BCSC to its communities.Effectively collaborates with staff peers and contractors to meet BCSC goals and further success.Demonstrates proficiency in delivering age-specific care.Complies with all policy and procedures of BCSC.And other tasks, as assigned.Education & Experience: Degree and diplomas: Master’s Degree from an accredited school of Social Work. LCSW preferred.Certificates, Licenses: Verification of completion of CPR (BLS) and first aid training. Must have a California driver’s license. TB screening and successful completion of a health examination by a health care provider is required for employment.Experience: At least one year of Supervisory experience and 3-5 years of  Social Work experience.Skills/Aptitudes: Ability to work effectively in a team environment. Age specific competency in working with the elderly. Knowledge of social work principles and practices, including case management and counseling techniques. Oversees care authorization, service utilization, and compliance with regulatory and organizational standards. Collaborates with interdisciplinary team (IDT) to align care decisions with participant-centered goals. Develop, implement, and continuously improve UM strategies, policies, and procedures. Align utilization practices with PACE model of care, focusing on frail, elderly populations. Monitor trends in service utilization and implement interventions to optimize quality outcomes and utilize resources appropriately for best outcomes. Provide clinical leadership for medical necessity determinations and authorization processes. Ensure appropriate level of care decisions across settings (home care, adult day health, inpatient, SNF). Review complex or high-cost cases and provide guidance on care planning. Ensure compliance with CMS, state Medicaid, and PACE regulatory requirements. Maintain audit readiness and oversee internal/external audits related to utilization management. Ensure documentation standards meet legal and accreditation requirements. Partner with physicians, nurses, social workers, and therapists within the IDT. Support care coordination and transitions of care to reduce avoidable hospitalizations. Participate in care planning meetings and case conferences. Analyze utilization data, including hospital admissions, readmissions, LOS, and service patterns. Develop and track KPIs and dashboards for UM performance. Lead initiatives to improve quality metrics and reduce unnecessary utilization. Lead, mentor, and evaluate UM staff including nurses and coordinators. Provide ongoing training on UM processes, clinical guidelines, and compliance requirements. Foster a culture of accountability, collaboration, and continuous improvement.Collaborate with contracted providers, hospitals, and post-acute facilities. Evaluate provider performance related to utilization and quality outcomes. Support contract negotiations with utilization considerations. Performs all other duties as assigned. Education & Experience:Degree and diplomas: Active RN license required; advanced degree (MSN, MHA, MPH) preferred.Certificates, Licenses: RN currently licensed by the California Board of Registered Nursing.  Verification of completion of CPR (BLS) and first aid training.  TB screening and successful completion of a health examination by a health care provider is required for employment. Must have valid California driver’s license. Experience: 7–10+ years of clinical and/or utilization management experience, preferably in geriatrics or managed care. 3–5+ years of leadership experience. Strong knowledge of CMS, Medicaid, and PACE regulations. Experience with value-based care and population health models.
