Ensure participation in the Interdisciplinary Team (IDT) and completion of the MDS portion within five (5) days, and subsequently on the 14th, 30th, 60th, and 90th days post-admission.Complete additional social service assessments, progress notes, and Person-Centered Plan (PCP) entries for all new admissions within seven (7) days.Ensure MDS documentation and IDT participation occur within fourteen (14) days of admission; complete any required updates within twenty-one (21) days.Educate staff and ensure that all social services documentation in medical records is accurate, descriptive, and reflective of services provided and the resident’s response to those services.Conduct interviews with residents and their families to obtain social histories. Engage residents and families in setting care objectives and individualized programming goals. Facilitate resident and family participation in care planning and coordinate interdisciplinary care conferences as needed.Identify and address social services deficiencies noted during surveys or consultant visits. Develop and implement corrective action plans and provide written documentation of such plans to the Administrator.Participate in facility committees (e.g., Infection Control, Care Plan, IDT, Safety, Room Moves) as assigned by the Administrator. Implement and follow through on committee recommendations.Perform other related duties as required.Experience and Skills:Education: Bachelor’s degree in Social Work or a related field is required. Also, review the supervisory staff assigned for each shift, and check all call-ins for the shift and succeeding shifts. Replace absent personnel in accordance with the department’s staffing policy.Complete M.A.B. certification upon hire and annually thereafter.Ensure a comprehensive assessment is completed upon admission, every 12 hours, and whenever there is a change in behavior, including but not limited to assessing suicidal ideation and homicidal ideation.Ensure that documentation is complete in the medical record, including but not limited to: shift charting, patient/family teaching, individual and group attendances, and MD orders.Communicate with the covering provider and staff regarding changes in patient condition, problems, or clinical concerns. Share necessary information with unit staff.Involve the patient in identifying and effectively utilizing coping skills to maintain safety and improve overall mental and physical well-being.Receive reports from RNs on the prior shift, including changes in condition, scheduled discharges, pending intakes, pending laboratory orders, and safety concerns.Audit and monitor patient reactions to treatment and ensure appropriate documentation of the patient’s condition.Initiate the Patient Care Plans for all assigned patients. Each shift, and as warranted by changes in the patient’s condition, ensure that all care plans are current and reflect individualized patient needs and goals.Observe nursing and therapy personnel to ensure established safety procedures are followed in the use of equipment and supplies.Perform other related duties as needed to meet the needs of the organization and patients.Ability to work a 7 p.m. – 7 a.m. shift.Education and Experience:Must possess a current license to practice as a Registered Nurse (RN) within the State of California.Must have, at a minimum, six (6) months of experience as a Registered Nurse (RN) in an acute care medical-surgical or psychiatric facility. We serve more than 4,000 seniors across our San Fernando Valley campuses, delivering personalized care tailored to each individual’s needs, from independent living to specialized healthcare programs.The Auerbach Geriatric Psychiatry Unit at LAJH is a leading facility for short-term psychiatric care, offering compassionate, specialized treatment tailored to the unique needs of seniors through a nationally-acclaimed model of high-quality care.The responsibilities of the Certified Nursing Assistant include, but are not limited to: Responsibilities:Assist residents with ambulatory training and mobility support.Participate in the feeding program, implementing structured techniques.Engage in the Activities of Daily Living (ADL) program.Demonstrate skill and tact in resident interaction and communication.Review care plans to ensure appropriate restorative nursing care is provided.Observe and report any pressure areas, skin breakdowns, injuries (including those of unknown origin), and skin tears to prevent decubitus ulcers. Inform the Nurse Supervisor/Charge Nurse of any changes in the resident’s condition.Monitor and report any changes in room temperature, ventilation, lighting, and ensure the room is prepared for receiving the resident (e.g., bed made, name tags displayed, admission kit available).Record all required entries in a timely manner on flow sheets, such as meal percentages, bowel movements, etc., using authorized abbreviations.Inventory and label resident's personal possessions as instructed.Assist residents with daily dental and oral hygiene care, including brushing teeth/dentures and special mouth care.Provide assistance with bath functions (bed bath, tub or shower bath) as directed.Assist with hair care (combing, brushing, shampooing) and nail care (clipping, trimming, and cleaning fingernails/toenails). Shave male residents as needed.Weigh and measure residents, and record vital signs (temperature, pulse, respiration).Participate in fire/safety programs, First Aid/CPR training, and attend a minimum of 75% of monthly meetings and in-services to maintain Certified Nursing Assistant certification (48 hours every two years).Follow established safety protocols in all duties, using necessary safety equipment (e.g., gait belt, mechanical lifts) when lifting or moving residents. Report hazardous conditions and equipment issues to the Nurse Supervisor/Charge Nurse immediately. Demonstrate proper body mechanics/ergonomics in all work practices.Experience and Skills:Current CNA certification issued by the State of California is required.Experience in a Skilled Nursing Facility, particularly with residents suffering from Alzheimer’s or dementia, is a plus.Verification of CPR (BLS) and First Aid certification is required.Background clearance, TB screening, and successful completion of a health examination by a Los Angeles Jewish Health Nurse Practitioner or M.D. are required.Join Los Angeles Jewish Health and make a meaningful impact in the lives of seniors in our community.
