We serve more than 4,000 seniors across our San Fernando Valley campuses, delivering personalized care tailored to each individual’s needs, from independent living to specialized healthcare programs.The responsibilities of the Director of Social Services include, but are not limited to: Responsibilities:Plan, develop, organize, implement, evaluate, and direct the social service programs of this facility.Assume the authority, responsibility and accountability of directing the social service department. Delegate authority, responsibility and accountability to other responsible department personnel.Review department policies and procedures annually and participate in making recommended changes. Assist in the orientation program for Residents/personnel in accordance with established policies and procedures.Update social service forms to ensure compliance with regulations and documentation requirements (e.g. MDS 3.0 and Title 22).Serve as liaison between JEK and providers such as Hospice, AGPU, psychiatrists and psychologists.Ensure documentation, including assessments, are completed within the required timeframe for all assigned to case load.Educate staff and assure all medical charts' social services entries are informative and descriptive of the services provided and of the Resident response to provided services.Complete behavioral management assessment tool for residents with psychotropic medications on a timely basis.Interview resident/families to obtain social history. Involve the resident/family in planning objectives, individualized programming and goals for the resident. Encourage the resident/family to participate in the development and review of his/her plan by facilitating residents/families and coordinate interdisciplinary resident/family conferences when necessary.Provide discharge planning services and make referrals as needed for the residents and their families.Work with the facility's consultants as necessary and implement recommended changes as required.Develop, implement, and maintain an ongoing quality assurance program for the social services department.Collaborate with admissions on an as needed basis regarding bed availability.Participate in facility surveys (inspections) made by authorized government agencies.Review and develop a plan of correction for social service deficiencies noted during survey inspections and provide a written copy of such to the Administrator.Supervise social service department as to their responsibility to provide ancillary services to all residents.Assume role of liaison with Omsbudsman ‘s office and assist Administrator as designated to conduct abuse allegation interviews and complete and submit necessary documentation to appropriate government agencies (i.e. SOC 341 reports to Adult Protective Services).Assume role of designated Liaison for Coordination with Hospice.Experience and Skills: Current LCSW license in the state of California required.Minimum of two years’ experience in a long-term care or skilled nursing facility.Supervisory experience preferred.Strong understanding of regulatory requirements (Title 22, etc.).Excellent communication, organizational, and problem-solving skills.Ability to work collaboratively with healthcare professionals, residents, and families.Join Los Angeles Jewish Health and make a meaningful impact in the lives of seniors in our community. Also, review the supervisory staff assigned for each shift, and check all call-ins for the shift and succeeding shifts. Replace absent personnel in accordance with the department’s staffing policy.Complete M.A.B. certification upon hire and annually thereafter.Ensure a comprehensive assessment is completed upon admission, every 12 hours, and whenever there is a change in behavior, including but not limited to assessing suicidal ideation and homicidal ideation.Ensure that documentation is complete in the medical record, including but not limited to: shift charting, patient/family teaching, individual and group attendances, and MD orders.Communicate with the covering provider and staff regarding changes in patient condition, problems, or clinical concerns. Share necessary information with unit staff.Involve the patient in identifying and effectively utilizing coping skills to maintain safety and improve overall mental and physical well-being.Receive reports from RNs on the prior shift, including changes in condition, scheduled discharges, pending intakes, pending laboratory orders, and safety concerns.Audit and monitor patient reactions to treatment and ensure appropriate documentation of the patient’s condition.Initiate the Patient Care Plans for all assigned patients. Each shift, and as warranted by changes in the patient’s condition, ensure that all care plans are current and reflect individualized patient needs and goals.Observe nursing and therapy personnel to ensure established safety procedures are followed in the use of equipment and supplies.Perform other related duties as needed to meet the needs of the organization and patients.Ability to work a 7 p.m. – 7 a.m. shift.Education and Experience:Must possess a current license to practice as a Registered Nurse (RN) within the State of California.Must have, at a minimum, six (6) months of experience as a Registered Nurse (RN) in an acute care medical-surgical or psychiatric facility. Ensure participation in the Interdisciplinary Team (IDT) and completion of the MDS portion within five (5) days, and subsequently on the 14th, 30th, 60th, and 90th days post-admission.Complete additional social service assessments, progress notes, and Person-Centered Plan (PCP) entries for all new admissions within seven (7) days.Ensure MDS documentation and IDT participation occur within fourteen (14) days of admission; complete any required updates within twenty-one (21) days.Educate staff and ensure that all social services documentation in medical records is accurate, descriptive, and reflective of services provided and the resident’s response to those services.Conduct interviews with residents and their families to obtain social histories. Engage residents and families in setting care objectives and individualized programming goals. Facilitate resident and family participation in care planning and coordinate interdisciplinary care conferences as needed.Identify and address social services deficiencies noted during surveys or consultant visits. Develop and implement corrective action plans and provide written documentation of such plans to the Administrator.Participate in facility committees (e.g., Infection Control, Care Plan, IDT, Safety, Room Moves) as assigned by the Administrator. Implement and follow through on committee recommendations.Perform other related duties as required.Experience and Skills:Education: Bachelor’s degree in Social Work or a related field is required. Records participant assessments, reassessments and care delivered in the medical record.Reviews all necessary documentation and coordinates with other disciplines as needed for bi-annual LOC audit preparation.Remains attentive to participants’ health status and social functioning; assists with identification of overt problems/needs and sets priorities; treats problems requiring immediate referral and/or follow-up; and interprets and records latest diagnostic results, as indicated. Attends Interdisciplinary Team meetings and other meetings as assigned by the RN Clinic Supervisor. In coordination with the Interdisciplinary Team and as assigned by the Clinic Supervisor RN, helps develop a Plan of Care based on nursing process that incorporates immediate and continuing care needs and includes the participant and family perspective. Provides care that conforms to accepted practice standards and age-related characteristics and needs of participants, as assigned by the Clinic Supervisor RN. Provides direct care as assigned by the Clinic Supervisor RN, including, but not limited to: wound care, IV therapy, blood sugar checks, IM injections, vital sign checks and/or follow-up on an episodic problem previously identified by the Primary Care Physician, such as Congestive Heart Failure, chest pain, dehydration, edema, urinary tract infection or other health problems.Consults with and educates the participant, family members and/or caregiver regarding the disease process, self-care techniques, prevention strategies and medical interventions, as indicated.Provides the Primary Care Physician with a summary of health findings and any changes in a participant’s health condition.Ensures that all tests, lab work and diagnostic studies ordered by the Primary Care Physician have been carried out, as assigned by the RN Clinic Supervisor.Coordinates all medication for BCSC, including the stocking of non-prescription medications, the ordering of prescription medications, the administering of medication in the PACE Center, and the monitoring of participant compliance. Arranges for delivery of needed medications and supplies to the participant’s home.Prior to the use of physical or chemical restraints, conducts an assessment and documents in the medical record that participant restraint alternatives have been attempted and obtains a physician order for restraints. Obtains family consent or refusal for restraint use. Monitors and documents that the participant has a specific medical symptom that cannot be addressed by another less restrictive intervention and a restraint is required to treat the medical symptom, protect the participant’s safety, and help the participant achieve his/her highest level of physical or psychological well-being. Recognizes inappropriate participant care management or service delivery by other Interdisciplinary Team members and addresses directly and/or refers to the RN Clinic Supervisor immediately.Identifies emergency situations and takes appropriate action, including coordinating direct hospital admissions from Clinic to hospital as necessary.Understands and demonstrates respect for participant rights and refers ethical issues to the BCSC Ethics Committee.Coordinates with the Home Care Coordinator to support participant care at home.Experience and Skills:Degree and Diplomas: Graduate of an accredited school of professional nursing; a BS in Nursing is preferred.Certificates & Licenses: Currently licensed by the California Board of Registered Nursing. TB screening and successful completion of a health examination by a healthcare provider are required for employment. Join our team of dedicated professionals committed to providing exceptional care to our community.Your responsibilities include, but are not limited to:RESPONSIBILITIES:Coordination with a clinical instructor, including regularly scheduled meetings to enhance your skills and provide excellent care.Communication with the clinical instructor and director of rehabilitation regarding any support needs.Participation in both direct and indirect learning experiences with your clinical instructor.Dedication to learning and progressing your skills as a new therapist.Screening and evaluating residents, developing appropriate care plans.Under a physician’s direction, providing direct patient/resident treatment based on the established plan of care.Planning, directing, or conducting rehabilitative treatment programs to restore communicative efficiency for individuals with communication, swallowing, or cognitive impairments.Administering, scoring, and interpreting specialized hearing and speech tests.Instructing residents, families, and caregivers on monitoring speech and practicing new skills.Completing required forms and documentation in accordance with company policy and state and/or federal regulations.Maintaining patient records accurately and in a timely manner.Devising special tools and treatments for the specific needs of patients.EXPERIENCE AND SKILLS:Master of Science degree in Speech-Language Pathology.Licensed in Speech-Language Pathology as required by state law.SUPERVISORY RESPONSIBILITIES:May supervise Rehab Aides and others involved in the program.PHYSICAL DEMANDS AND ENVIRONMENT:Shift work may be required.Employee is frequently required to stand, walk, use hands or fingers, reach with hands and arms, feel, talk, and hear.Employee may occasionally assist in lifting and/or transferring patients.Employee may be occasionally exposed to blood or other body fluids, fumes, airborne particles, and toxic or caustic chemicals.Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Join Los Angeles Jewish Health and make a meaningful impact in the lives of seniors in our community.
