The responsibilities of the Activities Team Leader include, but are not limited to:
Responsibilities:
- Plan, develop, organize, implement, evaluate, and direct the activity programs of this facility.
- Assist in the development, administering, and coordinating of department policies and procedures and interpret the department's policies and procedures to employees, residents, visitors, government agencies, etc.
- Involve residents and families in planning facility activity programs.
- Assist in arranging transportation to other facilities when necessary.
- Assist in the review and updating of departmental job descriptions at least annually.
- Review and evaluate the department's work force and make recommendations to the Administrator.
- Review and develop a plan of correction for activity deficiencies noted during survey inspections and provide a written copy of such report to the Administrator.
- Participate in facility surveys (inspections) made by authorized government agencies.
- Participate in discharge planning, development and implementation of activity care plans and resident assessments and ensure all charted activity progress notes are informative and descriptive of the services provided.
- Participate on various committees of the facility (i.e., Infection Control, Care Plan, IDT, Safety, etc.) as appointed by the Administrator.
- Meet with activity personnel, on a regularly scheduled basis, and solicit advice from inter‑department supervisors concerning the operation of the department, assist in identifying and correcting problem areas, and/or the improvement of services.
- Participate in the Quality Assessment & Assurance Committee by developing and implementing appropriate plans of action to correct identified deficiencies.
- Others as deemed necessary and appropriate, or as may be directed by the Administrator.
Experience and Skills:
- Bachelor's preferred, a combination of years of experience as a social worker or activity director may be acceptable.
- Supervisory experience preferred.
- Experience in dementia/memory care and Alzheimer’s preferred.
