The Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE), provides high-quality, comprehensive healthcare services. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.
The responsibilities of the Level of Care R.N. include, but are not limited to:
Responsibilities:
- Conducts initial home visits to perform nursing evaluations for Level of Care assessments.
- Completes initial and annual Level of Care assessments and submits them to the State for approval; manages corrections and updates as needed.
- Coordinates with the Home Care Coordinator to support participant care at home.
- Monitors participants’ health status and social functioning, identifies problems or needs, sets priorities, addresses issues requiring immediate referral or follow-up, and interprets and records diagnostic results as necessary.
- Attends Interdisciplinary Team meetings and other meetings as assigned by the RN Clinic Supervisor.
- In collaboration with the Interdisciplinary Team and as directed by the Clinic Supervisor RN, helps develop a Plan of Care that incorporates both immediate and ongoing care needs, integrating the perspectives of the participant and their family.
- Provides care in accordance with accepted practice standards, considering the age-related characteristics and needs of participants, as assigned by the Clinic Supervisor RN.
- Delivers direct care as assigned by the Clinic Supervisor RN, including, but not limited to: wound care, IV therapy, blood sugar checks, IM injections, vital sign monitoring, and follow-up care for issues previously identified by the Primary Care Physician (e.g., Congestive Heart Failure, chest pain, dehydration, edema, urinary tract infection, or other health concerns).
- Records participant assessments, reassessments, and care provided in the medical record.
- Consults with and educates the participant, family members, and/or caregivers regarding disease processes, self-care techniques, prevention strategies, and medical interventions as needed.
- Provides the Primary Care Physician with a summary of health findings and any changes in the participant’s condition.
- Ensures that all tests, lab work, and diagnostic studies ordered by the Primary Care Physician are completed, as assigned by the RN Clinic Supervisor.
- Coordinates all medication management for BCSC, including stocking non-prescription medications, ordering prescription medications, administering medication in the PACE Center, and monitoring participant compliance. Arranges for delivery of necessary medications and supplies to the participant’s home.
Experience and Skills:
Degree and Diplomas: Graduate of an accredited school of professional nursing; a BS in Nursing is preferred.
Certificates & Licenses: Currently licensed by the California Board of Registered Nursing. TB screening and successful completion of a health examination by a healthcare provider are required for employment. A valid California driver's license is also required.
Experience: At least two (2) years of experience as an RN, preferably in home or community health care; at least one (1) year of experience working with frail and elderly populations.
Skills & Aptitudes: Nursing knowledge and skills required to treat participants and manage complex clinical situations. Ability to work independently and collaboratively within an Interdisciplinary Team.
