The Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE), provides high-quality, comprehensive healthcare services. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.

The responsibilities of the Home Care RN include, but are not limited to:

Responsibilities:

  • Using the nursing process, provides semi-annual and annual assessments of assigned participants to evaluate home care needs. Records all assessments, reassessments, and care in the electronic medical record.
  • Conducts reassessments to determine ongoing needs and develops and implements a therapeutic Plan of Care with recommendations at least quarterly and whenever a participant's condition changes. Presents and coordinates the Plan of Care with the Interdisciplinary Team.
  • Conducts home visits as needed to assess medical status and needs as recommended by the Interdisciplinary Team.
  • Coordinates and implements all home care services, including Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, and Home Care Services, to ensure that quality services meet participants' needs.
  • Completes a Level of Care Assessment Tool (LOCAT) for all participants residing in contracted facilities.
  • Supervises home care for participants in board-and-care assisted living, residential care, and other supportive housing arrangements according to BCSC standards. Works with supportive housing staff to ensure effective communication and coordination of care.
  • Works closely with the Director of Quality and Compliance to oversee all activities related to quality assurance in Home Care.
  • Works closely with the Center Manager and Director of Quality and Compliance to receive, document, and resolve reports of participant rights violations, grievances, appeals, and other issues related to service and quality of care.
  • Actively participates in Interdisciplinary Team meetings.
  • Assists in the Clinic as assigned.
  • May provide direct care, including but not limited to wound care, IV therapy, blood sugar checks, IM injections, vital sign checks, and/or follow-up on episodic issues previously identified by the Primary Care Physician, such as Congestive Heart Failure, chest pain, dehydration, edema, urinary tract infection, or other health problems.
  • Available to cover for the LOC RN during sick/vacation days or when a second RN is needed to perform LOCs to maintain program enrollment.
  • Bilingual in Spanish is a plus.

Experience and Skills:

Degree and Diplomas: Graduated from an accredited school of professional nursing.

Certificates & Licenses: Licensed as a Registered Nurse (RN) through the California Board of Registered Nursing. Verification of completion of CPR (BLS) and first aid training. TB screening and successful completion of a health examination by a healthcare provider are required for employment. Must have a valid California driver’s license.

Experience: At least two years of experience as an RN, preferably in home or community health care. At least one year of experience working with a frail elderly population.

Skills & Aptitudes: Effective communication skills, both written and verbal, are required. Accuracy, attention to detail, and consistent follow-through are required. Willingness to work within the framework of a multidisciplinary team in the BCSC model of care.
