Job Functions:

As the Registered Dietitian, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties.

Administrative Functions:

  • Evaluates each participant’s nutritional status, nutrient requirements, food tolerance and resources for meal preparation. Conducts an initial and bi-annual nutrition assessment of each participant (on caseload) in order to determine dietary needs. Develops and implements a nutritional Plan of Care with recommendations for nutritional prescriptions, meals, and assistance with meal preparation and/or home-delivered meal care at least quarterly and when a participant’s condition changes. Presents the Plan of Care to the Interdisciplinary Team.
  • Maintains current written progress notes and other documentation on the participant Medical Record including signed and dated documentation for all nutrition/dietary related services performed the day the service is provided and quarterly progress reports.
  • Recommends meal composition and dietary requirements for each participant in coordination with the Interdisciplinary Team and in compliance with the Primary Care Physician’s orders.
  • Works closely with the Personal Care Attendants in the home and Center to monitor each participant’s eating habits. When appropriate, suggests modifications that best suit participant preferences and nutritional needs.
  • Provides special diets as required and in consultation with the Primary Care Physician.
  • Provides nutrition education to participants and family members/caregivers as ordered by the Primary Care Physician or Nurse Practitioner.
  • Provides nutrition education to Interdisciplinary Team members as needed and actively participates in Interdisciplinary Team meetings.
  • Works collaboratively with BCSC IDT in the implementation of participant education.
  • Participates in care coordination with SNF’s and ALF’s staff where participants reside and/or are in rehabilitation and attends assigned care coordination meetings.
  • Collaborates with outside agencies in the community to provide meal delivery services as needed.
  • As a member of the Inter-Disciplinary Team (IDT) will participate in IDT meetings and fulfill responsibilities as assigned.
  • Other duties as required.

Education and Experience:

  • Degrees and diplomas Bachelors Degree from an Academy of Nutrition and Dietetics accredited institution with a major in Nutrition; Master’s Degree preferred. Currently credentialed and licensed through the Academy of Nutrition and Dietetics.
  • Experience: At least one year experience as a Dietitian working with the frail or elderly or if less than 1-year appropriate training meeting industry standards of competency.
  • Skills/Aptitudes: Must be a health educator, able to think analytically and provide care to the frail elderly. Good working knowledge of clinical dietetics.
  • Bilingual, Spanish, preferred but not required.

Specific Requirements

  • Must be able to read, write, speak, and understand the English language.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, participants, visitors and the general public.
  • Must be knowledgeable of medical terminology, laws, and regulations as they pertain to long‑term care.
  • Must possess the ability to work harmoniously with other personnel.
  • Must possess the ability to minimize waste of supplies, misuse of equipment, etc.
  • Must be able to follow written and oral instructions.
  • Be knowledgeable in computers, data input and output.
  • Must be able to relate information concerning a participant’s condition.
  • Must not pose a direct threat to the health or safety of other individuals in the workplace.
