The Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE), provides high-quality, comprehensive healthcare services. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.

The responsibilities of the BCSC Social Worker include, but are not limited to: 

Responsibilities:

A BCSC Social Worker is expected to:

  • As a member of the Inter-Disciplinary Team (IDT) will participate in IDT meetings and fulfill responsibilities as assigned including initial assessments, care planning and on-going re-assessments of participant care. Duties may include participation as a proxy to provide a briefing to the IDT members for care planning and discussion of a specific participant.
  • Completes participant home visits, as required.
  • Maintains current written progress notes and other documentation on the participant Medical Record including signed and dated documentation for all services and quarterly progress reports.
  • Provides individual and family counseling and develops and leads group counseling sessions, as required.
  • Refers participants and families to appropriate community services and acts as liaison and/or advocate with community organizations for participants.
  • Works closely with participants and family members of caregivers during the disenrollment process, including assisting with referrals for primary care and community services.
  • Encourages effective relationships among staff to support the development and maintenance of a cohesive team.
  • Consults with and advises other staff regarding the relationship of social, emotional, and cultural factors to health and medical care, and to the availability of social services in the community.
  • Participates in program and policy development of BCSC Social Work.
  • Understands fiscal issues and is appropriately involved in meeting utilization targets.
  • Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to the Center Manager.  Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors and others.
  • Consistently meets or exceeds BCSC targets for productivity, customer service, quality assessment, and performance targets.
  • Continually seeks better ways for delivering services and communication with participants.
  • Understands the importance of community involvement and participates as appropriate in activities that link BCSC to its communities.
  • Effectively collaborates with staff peers and contractors to meet BCSC goals and further success.
  • Demonstrates proficiency in delivering age-specific care.
  • Complies with all policy and procedures of BCSC.
  • And other tasks, as assigned.

Experience and Skills:

  • Degree and diplomas: Master’s Degree from an accredited school of Social Work.
  • Certificates, Licenses: Verification of completion of CPR (BLS) and first aid training. Must have a California driver’s license. TB screening and successful completion of a health examination by a health care provider is required for employment.
  • Experience: At least one year social work experience, preferably in health care, and minimum one-year experience working with a frail or elderly population.
  • Skills/Aptitudes: Ability to work effectively in a team environment. Age specific competency in working with the elderly.  Knowledge of social work principles and practices, including case management and counseling techniques.  Working knowledge of community services for the elderly and their families.
