The responsibilities of the Nurse Practitioner include, but are not limited to:
Responsibilities:
- Through campus licensed community care clinic, provide primary care to specific residents under the medical direction and supervision of the residents’ attending physicians or the Medical/Associate Medical Director of the facility with emphasis on assessment, illness prevention and health care management.
- Assist in modifying the treatment regimen to meet the physical and psychosocial needs of the resident, in accordance with established medical practices and the requirements of this state and the policies and goals of the facility.
- Interact and communicate with physicians, residents, family, staff and administration in the terms of the nursing care provided in the clinic.
- Assume the duties of the RN/P.A. Clinic Supervisor in her/his absence.
Administrative Functions:
- Facilitate resident admission and coordinate ongoing care.
- Assess clinical findings, consult with physicians, and initiate referrals as needed.
- Monitors and records progress, evaluates plan of care, and makes modifications accordingly.
- Provides initial evaluation in emergency situations.
- Manage chronic and minor health conditions within scope and in collaboration with the Primary Physician.
- Regulates and adjusts medications and treatments in conjunction with Primary Physician.
- Consults and collaborates with members of the health care team and with families in the assessment and management of health care problems (physical and emotional) of the residents.
- Educate residents and families on self-care practices (e.g., incontinence, diabetes, medication use).
- Maintain weekday clinic hours for residents with new or ongoing health concerns.
- Participates with medical staff on walking rounds (Medical Director, Fellows, and NP from Nursing units).
- Consult regularly with attending physicians and nursing leadership on resident care.
- Complete medical documentation (forms, reports, charting) for assigned residents.
- Support compliance with clinical policies, quality assurance, and infection control standards.
- Provide updates to medical leadership and administration on resident care.
- Assist in policy development for nursing services.
- Participate in regulatory surveys and inspections.
Medical Care Functions:
- Orient new residents and educate them and their families on care and rights.
- Perform comprehensive assessments and physical exams using standard clinical methods.
- Document findings and develop care plans in coordination with physicians.
- Order and interpret diagnostic tests to guide treatment.
- Provide therapeutic care and contribute to care planning.
- Diagnose and treat minor conditions and manage chronic diseases per protocols.
- Monitor and adjust treatment regimens; refer residents as needed.
- Deliver emergency and supportive care to maintain comfort and stability.
- Coordinate admissions, discharges, and transitions in care.
- Support rehabilitation and assist with mobility and emotional needs.
- Promote preventive health through education on nutrition, safety, and wellness.
Experience and Skills:
- Graduate of an accredited School of Nursing and current license in State of California.
- Certificate of completion of a GNP/FNP/NP program at an institution of higher education that offers a Masters Degree in Nursing or Nurse Practitioner certification by the California Board of Registered Nursing.
- Experience working with frail or elderly populations is preferred.
