The Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE), provides high-quality, comprehensive healthcare services. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.
The responsibilities of the Licensed Vocational Nurse (LVN) include, but are not limited to:
Responsibilities:
- Provides health education to participants and their families to support the participant's care plan.
- Teaches participants and/or caregivers about the purpose, proper administration, and potential side effects of medications.
- Participates in QAPI activities to contribute to a safer medication administration system as needed.
- Processes and implements all medication orders according to BCSC policies and procedures.
- Administers all medications following the 5 Rights: participant, medication, dosage, time, route, and frequency.
- Administers medications accurately via the prescribed route.
- Communicates all medication orders to the appropriate staff.
- Administers and distributes all medications appropriately.
- Continuously monitors the participant’s response to medication.
- Reports adverse effects to the Clinic RN and/or PCP.
- Monitors the participant’s ability to self-administer medications.
- Monitors the timely return of medisets and documents in the medical record.
- Maintains an adequate supply of OTC medications in the Clinic.
- Maintains controlled medications according to established BCSC policies and procedures.
- Documents the efficacy of PRN medications when administered in the PACE Center.
- Resolves medication-related issues promptly and independently.
- Orders medical and medication supplies as needed.
- Coordinates with the Home Care RN/MA & Transportation Coordinator regarding medication delivery, lab work, and special requests.
- Performs procedures according to established nursing guidelines, including but not limited to: administration of injectable medications, checking vital signs and pulse oximeter, chest PT & postural drainage, catheterization and catheter care, bladder irrigation, cold compress (hot compress by PT), enema, eye/ear care, reading PPD results, safe handling of sharps and/or hazardous waste, ted hose stockings, and wound care.
- Responds to phone calls, overhead pages, and emails in a timely manner.
- Provides clear, concise, and thorough documentation (including Clinic involvement and changes/needs of participants) in a timely manner.
- Communicates changes in the nursing care plan during team meetings and to home care staff.
- Offers expertise and suggestions to assist the IDT in formulating the participant’s care plan.
- Communicates effectively with all team members, participants, and families in a sensitive and patient manner.
- Observes and reports all suspected cases of participant abuse to the Social Worker and Center Manager.
- Provides effective management of participants who wander.
- Consults with and educates participants, family members, and caregivers regarding disease processes, self-care, prevention strategies, and medical interventions.
- Participates in educational workshops and committee/task force meetings of interest to foster professional growth.
Experience and Skills:
Degree and Diplomas: Graduate of an accredited nursing school.
Certificates, Licenses: Licensed as a Vocational Nurse in the State of California. Verification of completion of CPR (BLS) and first aid certification bi-annually. Valid California driver’s license. TB screening and successful completion of a health examination by a health care provider is required for employment.
Experience: One year nursing experience and a minimum of one year of experience working with a frail elderly population.
Skills/Aptitudes: Strong communication skills, both written and verbal, are required. Accuracy, attention to detail, and consistent follow-through are essential. Willingness to work within the framework of a multidisciplinary team in the BCSC model of care. Sensitivity to the health beliefs and needs of a multi-ethnic aging population.
