The responsibilities of the Director of Social Services include, but are not limited to: 

Responsibilities:

  • Plan, develop, organize, implement, evaluate, and direct the social service programs of this facility.
  • Assume the authority, responsibility and accountability of directing the social service department. Delegate authority, responsibility and accountability to other responsible department personnel.
  • Review department policies and procedures annually and participate in making recommended changes.   
  • Assist in the orientation program for Residents/personnel in accordance with established policies and procedures.
  • Update social service forms to ensure compliance with regulations and documentation requirements (e.g. MDS 3.0 and Title 22).
  • Serve as liaison between JEK and providers such as Hospice, AGPU, psychiatrists and psychologists.
  • Ensure documentation, including assessments, are completed within the required timeframe for all assigned to case load.
  • Educate staff and assure all medical charts' social services entries are informative and descriptive of the services provided and of the Resident response to provided services.
  • Complete behavioral management assessment tool for residents with psychotropic medications on a timely basis.
  • Interview resident/families to obtain social history.  Involve the resident/family in planning objectives, individualized programming and goals for the resident.  Encourage the resident/family to participate in the development and review of his/her plan by facilitating residents/families and coordinate interdisciplinary resident/family conferences when necessary.
  • Provide discharge planning services and make referrals as needed for the residents and their families.
  • Work with the facility's consultants as necessary and implement recommended changes as required.
  • Develop, implement, and maintain an ongoing quality assurance program for the social services department.
  • Collaborate with admissions on an as needed basis regarding bed availability.
  • Participate in facility surveys (inspections) made by authorized government agencies.
  • Review and develop a plan of correction for social service deficiencies noted during survey inspections and provide a written copy of such to the Administrator.
  • Supervise social service department as to their responsibility to provide ancillary services to all residents.
  • Assume role of liaison with Omsbudsman ‘s office and assist Administrator as designated to conduct abuse allegation interviews and complete and submit necessary documentation to appropriate government agencies (i.e. SOC 341 reports to Adult Protective Services).
  • Assume role of designated Liaison for Coordination with Hospice.

Experience and Skills: 

  • Current LCSW license in the state of California required.
  • Minimum of two years’ experience in a long-term care or skilled nursing facility.
  • Supervisory experience preferred.
  • Strong understanding of regulatory requirements (Title 22, etc.).
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work collaboratively with healthcare professionals, residents, and families.
