This position is responsible for the provision of spiritual support to patients and families in keeping with the patient/family’s desire and belief system. The Rabbi provides counseling and crises intervention in order to adequately address the spiritual needs of hospice patients and their families.

QUALIFICATIONS

Education:

Graduate of accredited seminary or school of theology or appropriate certification in hospital or pastoral ministry. Masters of Divinity preferred


Experience:

Experience working with patients and families dealing with life threatening illnesses and death.

Core Competencies:


Ability to be open, flexible, and ecumenical. Ability to provide spiritual sensitivity to patients and families of all faiths and religious affiliations. Knowledge of beliefs and practices of cultures related to death and dying. Excellent oral and written communication skills. Ability to work with dying. Patients and their families. Possesses a sympathetic attitude toward caring for the hospice patient and demonstrates positive communication skills in interacting with other members of the hospice team.

Other: Valid California driver's license, and auto insurance.


FUNCTIONS & RESPONSIBILITIES:



1. Reports changes in the patient's conditions and needs to the RN Case Manager or RN Team
Leader/Supervisor if unable to reach the RN Team Leader/Supervisor
2. Performs spiritual assessments to identify patient/ family needs, concerns or opportunities for spiritual growth within regulatory timeframes.
3. Coordinates Spiritual Care Plan with IDT to assure a comprehensive Plan of Care
4. Provides direct spiritual support to patients/ families, including meditation, prayer, counseling, sacred rituals and practices.
5. Develops and maintains collegial relationship with clergy of all faiths and contacts clergy or
appropriate representatives of patients/ families as indicated.
6. Coordinates with Spiritual Counselor preferred by patient/ family if they do not want SC from our agency and is able to report SC activities being met and who is providing. Regularly maintains relationship with the alternative SC as chosen by patient/family
7. Assists patient/family with arrangements and/or conducts funeral or memorial services as indi­cated and according to agency policy.
8. Participates in the IDT in reviewing and assessing the potential spiritual needs of all patients/ families whether spiritual services are being provided or not.
9. Assists with bereavement follow-up services in collaboration with Bereavement Coordinator, as appropriate.
10. Participates in on-call activities related to EOBs, attendance at time of death, to meet spiritual needs of patients/ families, as appropriate.
11. Maintains a monthly schedule of visits including identification of alternate activities and submits prior to subsequent month. Updates and submits updates, timely.
12. Completes documentation of assessments, visits, communications with families, etc. and submits with daily time card on a daily basis.
13. Completes documentation prior to leaving facilities and leaves a copy in the facility medical record.
14. Communicates with facility charge nurse regarding activities and concerns related to SC of patient and assures changes to SC section of POC is communicated to facility charge nurse, family and hospice IDT and documented appropriately.
15. Maintains continuing education through academic studies, seminars, and workshops.
16. Participates in appropriate professional organizations.
17. Attends and participates in in-services, staff meetings and case conferences.
18. Completes all documentation in an accurate and legible manner.
19. Complies with hospice policies and procedures, laws and regulations and standards of practice.
20. Observes for safety hazards in the patient's and organization's environments and takes appropriate steps to reduce or eliminate them.
