The Brandman Centers for Senior Care (BCSC), a Program of All-inclusive Care for the Elderly (PACE), provides high-quality, comprehensive healthcare services. We are a program of Los Angeles Jewish Health, renowned for providing quality care to vulnerable seniors.

Under the supervision of the RN Clinic Supervisor, the Home Care RN is responsible for the development and implementation of home care services for program participants.

Administrative/Care Functions:

  • Using the nursing process provides semi-annual assessments and annual assessments of assigned participants to evaluate home care needs.  Records all assessments, reassessments and care in the electronic medical record.
  • Conducts reassessments to determine ongoing needs and develops and implements a therapeutic Plan of Care with recommendations at least quarterly and when a participant's condition changes .Presents and coordinates the Plan of Care with the Interdisciplinary Team.  
  • Conducts home visits as needed to assess medical status and needs as recommended by the Interdisciplinary Team. 
  • Coordinates and implements all home care services to include Nursing, Physical Therapy, Occupational Therapy, Speech Therapy and Home Care Services to assure that quality services are provided to meet participants needs.
  • Completes a level of care assessment tool (LOCAT) for all participants residing in contracted facilities.
  • Supervises home care for participants in board and care assisted living, residential care and other supportive housing arrangements according to BCSC standards. Works with supportive housing staff to ensure effective communication and coordination of care.
  • Works closely with the Director of Quality of and Compliance to oversee all activities related to quality assurance of Home Care.
  • Works closely with the Center Manger and Director of Quality and Compliance to receive, document and resolve reports of participant rights violations, grievance, appeals and other issues related to service and quality of care.
  • Actively participates in Interdisciplinary Team meetings.
  • Assists in the Clinic as assigned.
  • May provide direct care that includes, but is not limited to wound care, IV therapy, blood sugar checks, IM injections, vital sign checks and/or follow-up on an episodic problem previously identified by the Primary Care Physician, such as Congestive Heart Failure, chest pain, dehydration, edema, urinary tract infection or other health problems.
  • Available to cover for the LOC RN for sick/vacation days or in the case of needing a second RN to perform LOC's to maintain enrollment in the program.
  • Bilingual Spanish

Education and Experience:

  • Degree and Diplomas: Graduated from a school of professional nursing.
  • Certificates, Licenses: Licensed as an RN through California Board of Registered Nursing.  Verification of completion of CPR (BLS) and first aid training.  TB screening and successful completion of a health examination by a health care provider is required for employment.   Must have a valid California driver’s license.  
  • Experience: At least two years as an RN, preferably in home or community health care; at least one year of experience working with a frail elderly population.  
  • Skills/Aptitudes: Effective communication skills, both written and verbal, required. Accuracy, attention to detail, and consistent follow through are required. Willingness to work within the framework of a multidisciplinary team in the BCSC model of care. Sensitivity to the health beliefs and needs of a multi-ethnic aged population.
