Memory Care Program Coordinator at Eisenberg Village, Goldenberg Ziman Special Care Unit

Eisenberg Village is hiring a full time Memory Care Program Coordinator to oversee the 96 bed Dementia care unit.

The Memory Care Program Coordinator’s responsibilities are but not limited to:

This Special Position will be responsible for the following: Creates Activity Program containing a balanced mixture of activities addressing cognitive, recreational, and activity of daily living (ADL) needs. Cognitive activities include, but are not limited, to arts, crafts, storytelling, poetry readings, writing, music, reading, discussion, reminiscences, and reviews of current events. Recreational activities include all socially interactive activities, such as board games and cards, and physical exercise. Pet Therapy is encouraged.

*Promote Self-care ADLs include cleaning, dusting, cooking, gardening, and yard work through programming. Residents must be allowed to perform self-care ADLs as a long as they are able to promote independence and self- worth.

*Leads/facilitates activities program that are enriching and in line with the standards and expectations of the community.

*Coordinates and collaborates with team leaders in the planning of logistics of activities/events.

*Train Memory Care staff in all aspects of activity programming.

*Develop activities with residents who choose not to participate in a large group activity will be provided at least one small group or one-on-one activity per day.

*Review each resident’s medical and social history, preferences, and dislikes, in determining appropriate activities for the resident. Activities will be tailored to the residents’ unique requirements and skills.

*Establish a monthly activities schedule in collaboration with Memory Care staff. Structured activities should occur at the same time and place each week to ensure a consistent routine within the Memory Care neighborhood.

*Delegate responsibility and accountability to Memory care unit staff as indicated by members’ individualized service plans.

*Charts resident and family teaching when provided, concerning condition of dementia/memory care, related needs, medications, etc.

*Develops service plans in collaboration with IDT, and documents accordingly and on a timely basis. Recommends changes and adjustments in level of care in collaboration with IDT.

*Assists Director of Health Services with assessment of potential Memory Care members and determination of level of care.

*Encourages resident and family participation in activities of daily living to increase their independence and self-esteem. Assists resident in learning self-help techniques.

*Maintains a safe work area free from hazards of fire, spills, litter, chemicals and equipment.

*Reports any problems to the Director of Health Services or Executive Director.

*Respects the rights of residents and their family members.

*Prepares and submits all paper work in a timely manner.

*Maintains medication technician certificate.

*Maintains inventory of activity supplies in the Memory Care Unit and completes orders with approval of Executive director.

*Practices infection prevention measures and techniques and uses universal precautions.

*Practices efficient use of supplies, materials. Safeguards the resident's private property.

*Respects the rights of residents and families.

*Maintains confidentiality of all resident information and resident records.

*Accepts all job assignments.

*Conducts self in a professional manner at all times to reflect favorably upon the Community.

*Attends all Community required training sessions to maintain professional competencies.

*Performs other duties as assigned.

Experience and Skills:

*Degree or certificate in Activities and Dementia

*Must have, as a minimum, 1 year of management experience in a dementia care environment.

*Experience working with memory care/dementia patients in a healthcare setting is preferred.

*Must be able to read, write, speak, and understand the English language.

*Must possess the ability to deal tactfully with personnel, members, visitors and the general public.
