Skirball Hospice $125,000 - $145,000Skirball Hospice & Palliative Care is an established hospice agency located in the San Fernando Valley that provides hospice & palliative care services throughout Los Angeles County. We are seeking a professional Hospice Director of Patient Care Services to join our team. This role is responsible for the clinical management of the hospice program and serves as a conduit between the Administrator, clinical staff, patients, families, and the community. This role assumes lead responsibility for patient advocacy; and carries out the organization's purposes in a manner consistent with the goals, objectives, and established policies. The position reports to the Executive Director.
The ideal candidate will be professional, compassionate, organized, knowledgeable, and well spoken. They should have strong hospice clinical skills, effective communication skills and demonstrate exceptional ability in training staff and providing education to staff, patients and caregivers.

QUALIFICATIONS:
- Current CA RN License.
- BSN or MSN preferred but not required
- Minimum of five years’ experience in hospice, with three year’s supervisory or administrative experience
- Certification in Hospice and Palliative Nursing preferred but not required
- Valid California driver's license with good driving record and auto insurance
- Must be able to lift 50 pounds
- TB, COVID-19 and health screening required
EXPERIENCE AND CORE COMPETENCIES:
- Must meet state and federal requirements for the position
- Robust knowledge and understanding of the clinical delivery of hospice care services
- Strong knowledge of hospice eligibility requirements as well as federal, state, local and regulations as applicable to hospice & palliative care.
- Exceptional oral and written communication and presentation skills, with the proven ability to effectively communicate with patients and caregivers, hospice team members, business associates, and prospective referral sources.
- Proven experience in leading, managing, and mentoring a clinical team
- Ability to organize, delegate, supervise and evaluate staff
- Comprehensive working knowledge of various computer software systems and EMRs
- Comprehensive working knowledge of the current healthcare environment
- Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
- Possesses cultural awareness and sensitivity
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:
- Oversees the delivery of patient care under the hospice's Medicare benefit and ensures continuity of care for all patients. Provides direct delivery of care as needed.
- Responsible for the oversight of the assessment and evaluation, intervention and documentation of patient and family needs
- Oversees the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities
- Assists the Executive Director in the development and management of the budget
- Oversees the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services
- Leads discussion of hospice IDT conferences assisting team to discuss and implement plans of care and updates according to regulatory requirements and standards of practice
- Determines the hiring, retention, discipline, counseling or termination of the hospice's clinical team members
- Serves as a resource and support system to staff in problem solving and determining appropriate interventions.
- Oversees requirements and standards are maintained by all team members (including productivity standards).
- Oversees that clinical documentation is completed in an accurate, timely and legible manner.
- Responsible for the agency’s QAPI program
- Responsible for investigative and responding to patient related concerns.
- Responsible for hospice program planning and development.
- Oversees the orientation, competency evaluations, 90 day and annual review of clinical team members
- Investigates problems and take corrective action
- Responsible for and oversees the delivery of in-services, staff meetings and case conferences
- Responsible for and oversees identifying educational opportunities and developing training tools for staff members
- Assists in the development, interpretation and enforcement of hospice policies and procedures
- Maintains communication with Administrator, LAJH leadership and business partners
- Participates in community education and public relations as needed and promotes hospice referrals in the health care community
- Enforces compliance with local, State and Federal laws and regulations
- Serves as the alternate administrator in the absence of the Executive Director
- Other duties as assigned
